How to have ‘Better Conversations’ to support self-management and patient activation.

 

Know Your Own Health has been working with the NHS in delivering health coaching as an intervention since 2013, collecting data and monitoring outcomes. 

Initially the heiQ and later the PAM were used as outcomes measures, alongside patient-reported outcomes and patient experience.  

From 2015, we began to look more closely at the relationship between health coaching as an intervention and patient activation.

This focused on how the Coaching Conversation Flow can be used to intentionally increase patient activation. Using the Coaching Conversation Flow specifically in this way is what is now known as the Structured Agenda-free Coaching Conversation (StACC).

Preliminary results of a 3 ½ year evaluation of the StACC model were presented at the Society of Academic Primary Care Conference in June 2022.  The formal evaluation will be published once submission of the paper and the peer review process has been completed.
